Thaw fish per package instructions.
Preheat oven to 425F.
In a small bowl, combine mayonnaise and garlic.
In another small bowl, mix breadcrumbs, cheese, and melted butter.
Place cod on a baking sheet sprayed with non-stick cooking oil.
Season fish with salt and black pepper to taste.
Spread garlic mayonnaise on one side of each fillet. Top each fillet with half the breadcrumb mixture, gently pressing to adhere the crumbs.
Bake for 15 - 20 minutes until fish is flaky and topping is golden brown.
Serve with lemon wedges.
